Clinical Coordinator for families living with Huntington's Disease
This is a pilot project studying the positive difference a Clinical Coordinator can make in the pathway of care for people with Huntington's Disease.
The Clinical Coordinator's role is to coordinate services between the client's GP, Specialists and all allied health people involved to provide the best possible support for families living with Huntington's Disease.
The Clinical Coordinator will work closely with Professor Tim Anderson (Neurologist) and Dr Sandy Macleod (Psychiatrist) to follow up their recommendations and monitor people's wellbeing within their own communities.
There are two key eligibility criteria for the Pilot Study which are:
- A diagnosis of Huntington's Disease as diagnosed by a neurologist or genetic counsellor
- The person referred is currently residing in the Canterbury area as covered by the Canterbury District Health Board (from Kekerengu in the north to Rangitata in the south and across to Arthurs Pass).
